Autism Alert Cards

To Inform Officials, Emergency Personnel,
and Other Individuals Encountering Someone
on the Autism Spectrum



There are several options available for Autism Alert cards. If you feel you need one and the agency with which you are connected doesn't have one available, or if you aren't connected to any agency, here are some other cards you can either obtain or use for ideas in making your own.

If you make your own card or obtain one from an independent source, it will not certify you as having been diagnosed, so you should consider including information somewhere on it pertaining to how someone might obtain verification of your diagnosis.

Aspie Designed
ASD / Asperger's Alert Card
Now available in four versions:
Asperger's Specific = Versions 1 + 2
General Autism Spectrum = Versions 3 + 4

Version 1: Predesigned card alerting: "I am on the Autism Spectrum" and clarifying with "I have Asperger's syndrome." That side includes immediately needed general info and directs the viewer to turn the card over for personal info. Reverse includes blank lines for cardholder's name, emergency contact info for two people of the cardholder's choice, info about the diagnosing agency or professional, and a note to unfold for further info. Inside, there is more information about the autism spectrum in general on one panel and, on the other panel, space to note the cardholder's personally relevant health concerns - both special issues / accommodation needs and other health issues.

Version 1




Version 2: Also a predesigned card, with the front alerting: "I am on the Autism Spectrum" and clarifying with "I have Asperger's syndrome." Splits a greater amount of general info between the front and back sides of the card. The front calls attention to issues of immediate concern and directs the viewer to turn the card over for more information. The back gives helpful suggestions to those encountering someone on the spectrum and directs the viewer to unfold the card for personal info. One inside panel has more information about the autism spectrum in general. The other inside panel includes blank lines for the cardholder's name, emergency contact info for two people of the cardholder's choice, info about the diagnosing agency or professional, and space to note the cardholder's personally relevant health concerns - both special issues / accommodation needs and other health issues. The upside of this version is that it keeps private information from being so immediately displayed and leaves more room on the outer sides for instructions that might be needed quickly. The down side is that there's less room forspecial issues / accommodation needs and other health issues. The general autism spectrum info can be deleted in a graphics program, however, to make more room for those who need it for listing further personal info. Some people may also want their personal information to be more immediately accessible, so Version 1 may suit them better.

Directions:

When printed out,
the entire content appears on one side of a sheet,
so printing doesn't have to be done in duplex.
Simply print, cut out, and fold,
with the alert message and
the name / contact info facing outward.

This card can be printed on ordinary paper
and is intended to be folded like a map:
The inside panels and
the panel with the "front" and "back"
first fold along the lines, accordion style.
Then the accordion is folded in half,
along the remaining line,
creating the two sides that can then be
viewed from inside a wallet window
in the carrier's ID case.

There is no fee to obtain this card.
Feel free to download and print out as needed,
either in color or black and white.
Personal info may either be hand-written
or typed by opening the image in a
graphics program to modify it.
Spectrumite Designed
Autism Identification Card

Fill in the blanks to make your personalized card. None of the information you enter into the text boxes to create your card is stored on the Website. The information is discarded after the card print page is displayed.

On one side, this card, when complete, provides the viewer with information about personal identification; emergency contacts; physicians; medical conditions; medications, complete with dosages and times; and allergies. The other side provides general information about autism that is the same on every example of this card.
